Declaration

  1. I have successfully passed my 10th/plus two course and am eligible for higher studies (Diploma /Degree/Skill Development/Vocational training courses/B Voc)
  2. I request you to give an admission for Para medical/Management/Computational intelligence / Skill Courses at a College/ Institute Vocational Training Centre/ITP/academy recognized by the Universities / Board/NGO/ Autonomous body in India.
  3. I undertake not to involve in any activities which will destroy the peaceful atmosphere of the campus and hostel of Mother’s Group of Institutions.
  4. I also hereby undertake that I have never been subjected to any kind of disciplinary action in any vocational training centers or any other educational institutions.
  5. I will not indulge in any kind of strike or protest which will compel the administration of Mother’s Group of Institutions to take or not to take any decision.
  6. I shall be liable to follow all the rules framed by the respective state and its universities/board/NGO/Autonomous body and also all the laws laid down by the Hon’ble courts.
  7. I fully agree to attend the above said courses and exams either online or offline.
  8. I request you to provide me add-on courses that have additional fees which will be beneficial to me in future.
  9. I am fully aware of the fact that if the information furnished by me in this application is false, or the documents submitted by me are fabricated, I have to face all the legal consequences resulting there from.
  10. I am fully aware that all receipts received from the institute must be kept.
  11. I am fully aware that I have to comply all the rules and regulations imposed by the management for the smooth functioning of the Institution/College//ITP/Academy/Vocational Training Centre.
  12. I am also aware that the Scholarship/Sponsorship will be availed only if I obtain 85% marks and 85% of Attendance in all the Semesters of the above-mentioned Course and all discipline.
  13. I am fully aware that I am liable to comply with all the directions issued by the management in connection with the training.
  14. In case I am applying at a particular institution/academy/Vocational Training Centre/ITP /college and if no seats available there, I may be admitted to another with the same affiliation.
  15. I agree that I have to pay the Entire Fees if in case I discontinue a course midway after making admission in a College / Institute / Academy/ITP / Vocational Training Centre.
  16. I am fully aware that exam, Uniform, Id Card, Practical & Internship fees are extra.
  17. In case I fail to remit fees in the college/institution/academy/vocational training centre /ITP, I will be liable to compensate Mother’s Institute of Paramedical & Management for the loss and hardships if any.
  18. I am fully aware that admission taken once cannot be cancelled after three days, and I agree that I have to pay the whole fees of the course in case I want a cancellation.
  19. I have chosen this course with my full of knowledge, interest and passion to acquire skills for private sector.
  20. I am fully aware that I will be eligible for Internship & Placement assistance only if I have secured 85% attendance and all the discipline.
  21. I am aware that there may be changes in the notice board every day so check the notice board every day and follow all the changes made in it.
  22. I am fully aware that I will not be allowed to stay in any hostel or home stay which is not recommended by the management.
  23. I have chosen this course for improving my skill and for development of my career, I have voluntarily chosen to join this course.
  24. I have taken admission after checking the website: www.mipm.co.in and reading the brochure carefully.
  25. I am fully aware that I have to pay fine for the default in payment of Fees.
  26. I am fully agree and convinced to the fee structure of the course I have selected.
  27. I am fully aware and agree that the fees once paid will not be refunded under any circumstances.
  28. I am aware and agree that I have signed this after carefully reading all the Terms and conditions regarding the admission.
  29. I have clear knowledge about Bvoc courses of UGC Approved Universities, educational system of Bvoc courses and vocational training courses of NGO Autonomous Body.
